address and a general information request form.
To download the various intake forms, simply scroll down
and click the appropriate button.
Click on the button to the left to download the client INTAKE FORM.
You may either fill it out online and print or print the form first and then
fill it out. After printing off the form, please bring it with you to your
first appointment.

INFORMATION FORM if there are those individuals whom you want
to have access to your file (e.g., attorney, certain family members, etc.).
Click on the button to the left if you are coming for a DENTON
COUNTY PROBATION DRUG / ALCOHOL SCREENING. After
printing off the form and filling it out, please bring it with you to the first
appointment.
